Admission Requirements

The following documents are needed for your child to be enrolled at Herman Warren Academy:

    Signed and Completed Admission Form

    Signed and Completed Indemnity Form

    Signed and completed Terms and Conditions

    Copy of Birth Certificate

    Copy of Child’s Immunization card

    Copy of Parents/ Guardian’s ID/Passport

    Proof of Payment of Registration and Fees
